RxSpan MD: a surgeon-led telehealth menu with every price out in the open

RxSpan MD publishes every price before you type a single letter of your name. Compounded semaglutide runs $249 a month on a month-to-month plan, injection or capsule, and compounded tirzepatide starts at $329. The number each card leads with, though, is the 12-month prepaid rate with a promo code already applied, billed as one upfront payment. The monthly figure sits above it in strikethrough gray.

It's a physician-led practice founded by Dr. John Diaz, a board-certified plastic surgeon, and it sells more than weight loss. The same storefront carries hair-loss, sexual-health, and longevity products. For GLP-1 buyers the draw is a rare combination: named compounding pharmacies, named prescribers with NPI numbers, and brand-name Wegovy, Ozempic, and Zepbound on the same page as the compounded versions.

How the pricing works

Pick a medication on the weight-loss page and a plan picker opens beneath it with four tiers for each compounded product: monthly, 3-month, 6-month, and 12-month. For semaglutide injection the monthly tier is $249, the 3-month tier takes about 12% off that, the 6-month tier about 20%, and the 12-month tier lands roughly 30% below it “with STARTNOW promo”. The 12-month tier is billed as one upfront payment covering the full year, and the card says it “Auto-renews”.

STARTNOW is a sitewide banner: “Save 10% on any order — use code STARTNOW.” Select the monthly plan and the total-if-prescribed line drops 10% below the $249 list price. Whether the discount carries into the second month isn't stated anywhere on the site. Assume it doesn't, and budget from the list price.

Tirzepatide follows the same shape: injection is $329 monthly, the 3-month and 6-month tiers trim roughly 6% and 12%, and the 12-month tier with the code lands about a quarter below the monthly rate. Capsules run $349 monthly and drop further on the 3-month plan than the injection does. The cheapest number on the whole menu is semaglutide capsules on a 12-month prepay, about 35% under the monthly rate.

The overview under the picker says the price “includes consult, medication, supplies, shipping, and support”. There's no membership fee and no separate consult charge, and plans are “Pre-paid monthly, quarterly, semi-annually, or annually”, with payment taken before anyone reviews your case: “Prescription is not guaranteed.” What happens to your money if you're declined isn't published, and the Terms of Service has no refund clause at all.

Against a category median of about $179, the monthly rate is mid-pack. The 12-month tiers are competitive, but only if you're sure you'll stay on the medication for a full year.

How we get the market median: $179 is the middle value of each program's lowest listed ongoing monthly price for compounded semaglutide (month to month, not a first-month promo, a prepaid plan or a price with a membership on top), across the 112 programs that publish one.

Medications, forms, and the dose question

All seven products sit on one page, four of them compounded: semaglutide injection, semaglutide capsules, tirzepatide injection, and tirzepatide capsules. Three are brand: Ozempic at $1,299, Zepbound at $1,069, and Wegovy at $1,349, each on a monthly plan only with no multi-month tier. RxSpan doesn't sell Mounjaro or the Wegovy pill, so the only oral options are the compounded capsules.

The compounded products list vitamin B12 as an added ingredient. The medication pages say dosing “is usually started low and increased gradually”, but RxSpan doesn't publish its own ladder in milligrams. If you want to know whether you'd move through the standard 0.25 mg to 2.4 mg semaglutide steps, or the 2.5 mg to 15 mg tirzepatide range, you'll have to ask the prescriber. Closer lab monitoring is the draw at GoodLife Health, whose primary care membership includes a lab panel every 90 days alongside brand-name GLP-1s, with the medication billed separately.

The price is flat across doses, so the 12-month rate you sign at is the rate you keep, on a starter dose and a maintenance dose alike. Dose-scaled competitors get expensive in month four; RxSpan doesn't.

The homepage also advertises a “Microdosing GLP-1” option. There's no microdosing product or price anywhere on the weight-loss menu, so treat that as a conversation with the provider, not a plan you can buy.

Dr. Diaz's own comparison page ranks the compounded capsules last for evidence, says there is no FDA-approved oral tirzepatide, and cites the trial doses behind each brand, which is unusually candid for a seller's page.

What signing up looks like

The intake starts at the plan picker, not after it. Choose a product, choose a tier, press Next, and the first screen asks for your first name, last name, email, and gender assigned at birth. That's where a window-shopper stops, because there's no anonymous quiz to test eligibility first.

The homepage calls it a 2-minute assessment in one place and says it “Takes about 5 minutes” in another. The How-It-Works section lists a “Virtual Consult” step, but the FAQ walks that back: “Not always. Many members are approved based on their assessment alone.”

After approval the prescription goes to one of four named 503A pharmacies: Belmar, Strive, Epiq Scripts, or Casa Pharma Rx. Which one fills yours isn't stated, and delivery is described only as “Free Expedited Delivery” with no day count published. Refills and check-ins run through the same provider, and the plan renews automatically at the term you chose. Support is “Unlimited 24/7”, with unlimited appointments and messaging included.

RxSpan says it serves “all 50 U.S. states”, with the caveat that specific services vary by state licensure.

Trust and safety

The trust picture is better than most compounded storefronts. The prescribers are named: Lion MD, led by Dr. Ana Lisa Carr and Dr. Kelly Tenbrink, both with NPI numbers on the site, and the pharmacies are named with phone numbers. The legal entity, RxSpan MD LLC, is in the Terms with a Beverly Hills address. A LegitScript seal sits in the footer, and there are no FDA warning letters against the company.

What's missing is the paperwork around your money. No refund policy, no statement about what a declined applicant gets back, and no explanation of how “Auto-renews” squares with “Cancel or change plans anytime”. The medication pages carry the standard compounded-drug disclosure: not FDA-approved, not reviewed for safety or effectiveness. RxSpan also doesn't say whether it takes insurance or HSA/FSA cards. Insured patients have another route in Knownwell, an obesity-medicine practice that accepts insurance for branded GLP-1s and runs clinics in four metro areas, though it doesn't post self-pay medication prices.

Who should choose it, and who should skip it

Choose RxSpan MD if you want the full price grid before giving anyone your email, if named pharmacies and named prescribers matter to you, and if you'd like to move between compounded and brand-name medication without switching providers. The 12-month capsule plan, about 35% under the monthly rate, is a low number for someone who already knows they'll stay the course.

Skip it if you're not ready to commit to a year, because the monthly rate is ordinary and the savings only arrive with prepay. Skip it if you need a refund policy in writing before you pay. And skip it if you want a clinic that only does weight loss, because the hair and sexual-health products share the same storefront.
