Mounjaro

Mounjaro is tirzepatide labeled for type 2 diabetes. It is the same molecule, in the same strengths, as Zepbound — the difference is entirely in what the label says the drug is for. Mounjaro’s indications are improving blood sugar control in adults and in children aged 10 and older with type 2 diabetes, and reducing the risk of heart attack, stroke and cardiovascular death in adults with type 2 diabetes at high risk for those events.

Tirzepatide activates both the GIP and GLP-1 receptors, and in type 2 diabetes that combination has outperformed semaglutide head to head. In SURPASS-2, A1C fell by 2.01, 2.24 and 2.30 percentage points at 5, 10 and 15 mg of tirzepatide versus 1.86 points on semaglutide 1 mg over 40 weeks, with weight reductions greater by 1.9 kg, 3.6 kg and 5.5 kg respectively. Those are diabetes-population numbers; the obesity comparison is a separate trial.

Dosing begins at 2.5 mg once weekly and increases in 2.5 mg steps after at least four weeks on the current dose, to a maximum of 15 mg in adults (10 mg in children). A missed dose can be taken within four days; after that, skip it. Keep at least 72 hours between two injections if you shift your dosing day. The KwikPen click chart covers device specifics.

Two things trip people up. First, using Mounjaro for weight loss without diabetes is off-label — permitted, but it shapes how a prior authorization gets reviewed. Second, switching between Mounjaro and Zepbound is not automatically seamless from an insurance standpoint even though the syringe contents are the same.
