Orforglipron

Orforglipron is a small-molecule GLP-1 receptor agonist taken as a daily tablet, sold in the United States under the brand name Foundayo. Every earlier GLP-1 medicine was a peptide — a short protein chain that either has to be injected or needs an absorption enhancer and a fasting window to survive the gut. Orforglipron is not a peptide at all. It is a conventional drug molecule that happens to fit the same receptor.

That distinction has three practical consequences. It can be swallowed with or without food, so there is no empty-stomach ritual. It is made by ordinary chemical synthesis, which is cheaper and far easier to scale than peptide manufacturing — the constraint that produced years of injectable shortages. And its elimination half-life is roughly 29 to 49 hours, so the drug clears in days rather than the five to seven weeks semaglutide takes.

Dosing is deliberately gradual: 0.8 mg once daily to start, increasing to 2.5 mg after at least 30 days, then 5.5 mg, with optional further steps to 9 mg, 14.5 mg and a maximum of 17.2 mg, each requiring at least 30 days at the prior level. Reaching the top dose takes roughly five months, which is slower than the injectable ladders and worth planning refills around.

The common misreading is that an oral agent must be gentler. The label carries the same boxed warning about thyroid C-cell tumors seen across the class, along with warnings for acute pancreatitis, gallbladder disease, severe gastrointestinal reactions and delayed gastric emptying before general anesthesia. Gastrointestinal side effects are the reason the escalation is spread across months.
