Foundayo in plain English
Foundayo is Eli Lilly’s brand of orforglipron, a once-daily GLP-1 receptor agonist in tablet form. Its label, published in August 2026, approves it alongside a reduced-calorie diet and increased physical activity to reduce excess body weight and keep it off in adults with obesity, or in adults with extra weight plus at least one weight-related condition. It is the first small-molecule, non-peptide GLP-1 agonist to reach the US market for weight.
The practical significance is in what the tablet does not require. Unlike oral semaglutide, Foundayo is taken with or without food — no empty-stomach window, no four-ounce water rule, no 30-minute wait before your coffee. Swallow one tablet whole, once a day, and that is the whole routine. Because it is manufactured by conventional chemical synthesis rather than peptide synthesis, it also sidesteps the capacity bottleneck that drove years of injectable shortages.
Dosing climbs slowly, in 30-day steps rather than the weekly-to-monthly steps used by the injections: 0.8 mg daily to start, then 2.5 mg after at least 30 days, then 5.5 mg, with optional further increases to 9 mg, 14.5 mg and a 17.2 mg maximum, each after at least 30 days on the prior dose. Reaching the top dose therefore takes months. Orforglipron’s elimination half-life is roughly 29 to 49 hours, far shorter than semaglutide’s week. Our week-by-week titration guide lays the ladder out.
The misconception to avoid is treating “a pill” as shorthand for “milder.” Foundayo carries the same boxed warning about thyroid C-cell tumors as the injectables, plus label warnings for pancreatitis, gallbladder disease and delayed gastric emptying before anesthesia.
Worth asking: whether the six-step escalation is realistic for your schedule and refill cadence; how the daily tablet compares on cost to a weekly injection in your plan; and whether your prescriber expects you to reach the maximum dose. See the Foundayo drug page and the three-way comparison.
