Ecnoglutide

Ecnoglutide is a once-weekly GLP-1 receptor agonist developed in China and described as cAMP signaling-biased — engineered to favor one downstream pathway from the receptor over others, on the theory that a biased signal may separate benefit from side effect. It is not FDA-approved and is not available in the United States.

Its phase 3 obesity trial, published in The Lancet Diabetes & Endocrinology in 2025, randomized 664 adults with overweight or obesity to ecnoglutide 1.2 mg, 1.8 mg, 2.4 mg or placebo. At week 40, least-squares mean weight change was −9.1%, −10.9% and −13.2% across the three doses versus +0.1% on placebo. Weight loss of at least 5% was reached by 77%, 84% and 87% of the dose groups against 16% on placebo.

Read the trial duration before comparing those numbers with anything else. Forty weeks is substantially shorter than the 68 and 72-week trials that produced semaglutide’s 14.9% and tirzepatide’s 20.9%, and weight loss on these drugs continues well past week 40. A cross-trial comparison at different time points is not a fair fight in either direction.

The wider significance is about supply and price rather than pharmacology. Ecnoglutide is one of several GLP-1 agents developed outside the United States and Europe, and a larger global field eventually means more manufacturing capacity and more competition. That is a slow-moving benefit, not one that changes anyone’s options this year.
