Zepbound in plain English

Zepbound is Eli Lilly’s brand of tirzepatide for weight, given as a once-weekly injection. Unlike the semaglutide products it activates two gut-hormone receptors rather than one: GIP and GLP-1. Its label covers reducing excess body weight and maintaining that reduction long term in adults with obesity or with extra weight plus a weight-related condition, and — distinctively — treating moderate to severe obstructive sleep apnea in adults with obesity.

SURMOUNT-1 enrolled 2,539 adults without diabetes and ran 72 weeks. Average weight change was −15.0% at 5 mg, −19.5% at 10 mg and −20.9% at 15 mg, against −3.1% on placebo. At the two higher doses, 50% and 57% of participants lost at least a fifth of their body weight. The sleep-apnea indication rests on SURMOUNT-OSA, where the apnea-hypopnea index fell by about 25 to 29 events per hour depending on whether participants were already using a CPAP machine — starting from a mean near 50 events per hour.

Dosing starts at 2.5 mg weekly for four weeks, which the label states is a starter dose and not a maintenance dose, then rises in 2.5 mg steps no sooner than every four weeks. Maintenance is 5, 10 or 15 mg for weight; sleep apnea uses 10 or 15 mg. Miss a dose and you have four days (96 hours) to take it before skipping. Our pen dose chart and titration planner handle the scheduling.

The mix-up here is Zepbound versus Mounjaro: identical molecule, identical strengths, different brand and different labeled use. Pharmacies and plans treat them as separate products, so a coverage denial on one says nothing about the other.

Ask your clinician whether a sleep study would strengthen your coverage case, what maintenance dose they are aiming for rather than just the starting one, and how they plan to protect muscle during a 20% loss. Our SURMOUNT-OSA breakdown and the Zepbound drug page go further.
