The Wegovy Pill (Oral Semaglutide 25 mg): Dosing, Results and Cost

The Wegovy pill is oral semaglutide, the same molecule as the Wegovy injection, pressed into a once-daily tablet. The FDA approved it in December 2025 as a new dosage form of Wegovy for adults with obesity, or overweight plus a weight-related condition, and for cardiovascular risk reduction ^[2]. It comes in four strengths (1.5 mg, 4 mg, 9 mg, and 25 mg), is taken first thing in the morning on an empty stomach with no more than 4 ounces of water, and reaches its 25 mg maintenance dose after 90 days ^[1]. In the 64-week OASIS 4 trial, adults on the 25 mg tablet lost a mean 13.6% of body weight versus 2.2% on placebo, a difference of 11.4 percentage points (95% CI −13.9 to −9.0) ^[3]. That is close to the 14.9% the 2.4 mg weekly injection produced in its own pivotal trial ^[5]. Self-pay through NovoCare Pharmacy runs $149 a month at the starting dose and $299 a month at the 9 mg and 25 mg doses as of September 2026 ^[9].

The short version

• It is Wegovy, not Rybelsus. Rybelsus (3, 7, and 14 mg) is oral semaglutide approved for type 2 diabetes; Wegovy tablets (up to 25 mg) carry the weight-management and cardiovascular indications ^[1][11].

What the Wegovy pill is

Semaglutide is a peptide, and peptides are digested if swallowed. The tablet gets around that by pairing semaglutide with an absorption enhancer, SNAC, which raises the pH around the tablet and lets the drug cross the stomach lining before it breaks down ^[8]. The label says absorption “predominantly occurs in the stomach” and that absolute bioavailability “is estimated to be approximately 1% to 2%” ^[1]. That is why the oral dose is 25 mg a day while the injection is 2.4 mg a week.

The exposure ends up similar. Per the label, the average steady-state semaglutide concentration is roughly 77 nmol/L on the 25 mg tablet and roughly 75 nmol/L on the 2.4 mg weekly injection ^[1]. A Novo Nordisk pharmacokinetic analysis found that 82.2% of OASIS 4 participants on the tablet landed in the same exposure range as STEP 1 participants on the injection ^[7]. FDA approval: when, and for what

Drugs@FDA lists Wegovy tablets under NDA 218316, approved December 22, 2025, as a “Type 3 - New Dosage Form” ^[2]. The tablet did not get its own brand; it was folded into the existing Wegovy label, which now covers both forms ^[1]. The tablet indications, quoted from that label:

WEGOVY tablets are indicated in combination with a reduced calorie diet and increased physical activity: to reduce the risk of major adverse CV events (CV death, non-fatal myocardial infarction, or non-fatal stroke) in adults with established CV disease and either obesity or overweight; to reduce excess body weight and maintain weight reduction long term in adults with obesity, or in adults with overweight in the presence of at least one weight-related comorbid condition.

Two gaps versus the injection matter. The tablet is approved for adults only; the adolescent weight-management indication and the MASH liver indication belong to the injection alone ^[1]. And the label's limitations of use say combining Wegovy tablets with any other semaglutide product or GLP-1 receptor agonist “is not recommended” ^[1], so the pill replaces the pen rather than adding to it.

Strengths and the 90-day dosing schedule

The label's Table 2 sets a four-step escalation, each step lasting 30 days, with 25 mg as the single recommended maintenance dose for both indications ^[1]. Unlike the injection, the tablet schedule has no lower maintenance rung.

Each strength ships as a bottle of 30 tablets ^[1], which is why every price is quoted per bottle. A missed tablet is simply skipped (“take the next dose the following day”), never doubled ^[1]. Patients already on the 2.4 mg injection can move straight to the 25 mg tablet one week after their last shot; going back, the label says to start 2.4 mg weekly the day after the last tablet, and for people who cannot tolerate 25 mg it suggests the 1.7 mg injection instead ^[1]. How to take the Wegovy pill (the empty-stomach rule)

Every instruction protects that 1% to 2% absorption. The label says to take it “once-daily on an empty stomach in the morning with water (up to 4 ounces)” ^[1]. In full:

1. Morning, before anything else. Wake up and take one tablet on an empty stomach ^[1].
2. Water only, no more than 4 ounces. “Do not take with other liquids besides water” ^[1]. Coffee counts as another liquid.
3. Swallow it whole. “Do not split, crush, chew or dissolve” ^[1].
4. Wait at least 30 minutes before eating, drinking anything else, or taking other oral medications ^[1]. Novo Nordisk's patient page shortens it to “Sip” then “Go” ^[10].
5. Keep it in the original bottle and skip any tablet that looks damaged or discolored ^[10].

The most-searched follow-up, “can I take the Wegovy pill at night,” has a plain answer: the label specifies the morning, and OASIS 4 dosed it that way ^[1][3]. A bedtime dose lands in a stomach that is rarely empty and has no trial behind it. How much weight people lost: the OASIS 4 trial

OASIS 4 (NCT05564117) is the trial the approval rests on. It randomized 307 adults without diabetes, with a BMI of 30 or higher or 27 or higher with a weight-related complication, two-to-one to oral semaglutide 25 mg or placebo, plus lifestyle counseling, for 64 weeks ^[3]. Mean baseline weight was 106 kg (about 234 lb). The estimated mean change in body weight at week 64 was −13.6% with the tablet versus −2.2% with placebo, an estimated difference of −11.4 percentage points (95% CI −13.9 to −9.0; P<0.001) ^[3]. The FDA label reports the same trial as Study 7 with a placebo estimate of −2.4% and a difference of −11.2 points (95% CI −13.6 to −8.8) ^[1]; the gap is two statistical models, not two trials.

The earlier OASIS 1 trial tested a 50 mg tablet in 667 adults and found −15.1% versus −2.4% at 68 weeks (estimated difference −12.7 points, 95% CI −14.2 to −11.3) ^[4]. Pill vs injection: is it as effective?

No trial has randomized people to the pill versus the pen, so every comparison is across trials run years apart in different populations. With that caveat: STEP 1 gave the 2.4 mg injection −14.9% versus −2.4% at 68 weeks, a difference of −12.4 points (95% CI −13.4 to −11.5), in 1,961 adults ^[5]. OASIS 4 gave the tablet −13.6% versus −2.2% at 64 weeks, a difference of −11.4 points ^[3]. The responder rates line up as well.

A Novo Nordisk-funded indirect treatment comparison of the two trials estimated the injection's edge at 1.01 percentage points of weight loss (95% CI −1.61 to 3.63), an interval that includes zero and sits inside the 5% margin the FDA treats as clinically relevant; the authors called the two forms “comparable” ^[6]. Read that with its funding in mind, but it agrees with the label's pharmacokinetics. The practical difference is adherence: a weekly shot taken with dinner is easier to keep up than a daily tablet with a 30-minute fasting window. Side effects of the Wegovy pill

The tablet's side-effect profile is the injection's. In OASIS 4, gastrointestinal adverse events were reported by 74.0% of participants on the tablet versus 42.2% on placebo ^[3]. The label says that in the tablet trial “the types and frequency of common adverse reactions were similar to those listed in Table 3,” the injection table, where the leading entries are nausea (44% versus 16% on placebo), diarrhea (30% versus 16%), vomiting (24% versus 6%), constipation (24% versus 11%), and abdominal pain (20% versus 10%) ^[1]. The tablet shares the class boxed warning: it is contraindicated with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2, and the pancreatitis, gallbladder, hypoglycemia, and kidney-injury warnings all carry over ^[1]. One caution is tablet-specific. SNAC and its metabolites pass into breast milk, and infants may clear it more slowly, so the label says breastfeeding “is not recommended during treatment with WEGOVY tablets”; the injection contains no SNAC ^[1]. And because the tablet must go down 30 minutes before any other oral medication, anyone with other morning pills should map out the schedule with their prescriber first.

What the Wegovy pill costs (September 2026)

NovoCare Pharmacy, Novo Nordisk's direct self-pay channel, prices the tablet by dose, and the bottle gets more expensive as you climb ^[9]. The list price before discounts is $1,349.02 per package, the same figure Novo publishes for the pen ^[10]. So the “starting at $149” headline covers one bottle; a full year on the label schedule comes to roughly $3,340 before any visit fees. Two other channels change the math. With commercial insurance that covers Wegovy, the savings offer brings the copay to as little as $25 a month, capped at $100 of savings, with government-plan members excluded ^[9]. How to get the Wegovy pill

The pill is prescription-only with the same eligibility as the pen: a BMI of 30 or higher, or 27 or higher with at least one weight-related condition, or established cardiovascular disease with obesity or overweight ^[1]. The prescription can be filled at a retail pharmacy with the savings offer or sent electronically to NovoCare Pharmacy for home delivery ^[9][10]. Several telehealth companies we review now carry the brand tablet, and their pricing differs more in structure than in headline. LifeMD lists the Wegovy pill from $149 a month for the medication on top of a $149 monthly program fee, brand products only. Sesame Care lists it at $149 a month for the 1.5 mg and 4 mg doses and $299 a month for 9 mg and 25 mg, plus its subscription. Maximus lists Wegovy tablets from $249.99 a month. Who the Wegovy pill suits, and who it does not

• A good fit: adults who meet the BMI criteria, want to avoid injections, and can take a tablet first thing every morning and wait 30 minutes before coffee ^[1].
• A good fit: people stable on the 2.4 mg pen who want to switch; the label allows a direct move to 25 mg one week after the last injection ^[1].
• Not approved: anyone under 18, and the label recommends against it while breastfeeding ^[1].
• Not a fit: people who cannot keep a fasting window or who found 25 mg intolerable; the label points them to the 1.7 mg injection ^[1].
• Not the same thing: compounded “oral semaglutide” drops, troches, and sublingual products are not Wegovy tablets and were not the product studied in the OASIS trials. See oral peptides: evidence vs hype.

Bottom line

The Wegovy pill delivers the injection's drug at the injection's blood levels and, on the trial evidence, close to the injection's weight loss: about 14% at 64 weeks against about 15% at 68 weeks for the pen, with the same gastrointestinal side effects ^[3][5]. What it asks in return is a strict morning routine and a self-pay price that doubles between the first bottle and the fourth.
