Compounded Semaglutide Dosage in Units: The Conversion, and Why It Goes Wrong

Compounded semaglutide is prescribed in milligrams and drawn with a syringe marked in units — two scales that are not interchangeable. Here is the arithmetic in both directions, the documented harm when it is done wrong, and the small errors that hide inside a correct calculation.

Compounded semaglutide is dispensed in a vial and drawn with an insulin syringe. Your prescription says milligrams. The syringe says units. Those two scales are not interchangeable, and nothing on the syringe tells you the difference — a unit is a fixed volume of 0.01 mL, so what it contains depends entirely on how concentrated your vial is. This is not a subtle distinction: a poison control center published a case series of patients who measured in the wrong scale and took several times their intended dose[1]. Semaglutide's own pharmacology then makes the mistake hard to walk back, because it is cleared over about a week[3].

Read your own label

Everything below is arithmetic. It is correct for whatever concentration you enter, and it cannot tell you what your vial contains. Only the pharmacy label states the concentration in mg/mL. If it is not printed there, do not estimate it — call the pharmacy. A vial labeled only with a total quantity, such as “5 mg”, does not state a concentration until you also know the volume it is dissolved in.

Why units and milligrams are not the same language

Insulin syringes are calibrated in units because insulin is standardized at 100 units per mL — the U-100 standard. The marks on the barrel are volume marks wearing an insulin label. Semaglutide has no unit strength at all; it is prescribed in milligrams, exactly as the brand pens are (0.25 mg, 0.5 mg, 1 mg, 1.7 mg, 2.4 mg). So when a compounded vial and an insulin syringe are put in the same box, the patient is quietly handed a units-to-milligrams conversion that the brand pen performed mechanically and invisibly.

A Wegovy or Ozempic pen has a dose selector. You dial the milligram dose the prescription names and the device delivers it; the volume involved is never your problem. Remove the pen and that hidden step becomes a manual calculation done at a kitchen table, usually once a week, often in poor light, sometimes by someone who has never used a syringe before.

The conversion

Milligrams = (units ÷ 100) × concentration in mg/mL

The ×100 and ÷100 assume a U-100 syringe, where 1 mL is 100 units. Check the barrel: a U-40 syringe or one graduated in milliliters changes the arithmetic.

Notice how far apart the columns are. A 1 mg dose is a full 1 mL syringe at one concentration and a fifth of that at another. Any answer to “how many units is 1 mg of semaglutide” that does not begin by asking about your vial is guessing.

That last row is the one worth staring at. One hundred units — a completely full standard insulin syringe — is 1 mg from a dilute vial and 5 mg from a concentrated one. Five milligrams is more than double the highest approved weekly weight-management dose of semaglutide.

The documented failure mode

In the case series reported to a poison control center, the pattern was consistent: patients drew the number they had been told, but on the wrong scale, and presented with nausea, vomiting and dehydration[1]. The errors were not exotic. They were the predictable result of asking people to convert between two unit systems using a device labeled for a third drug.

Semaglutide's kinetics remove any margin for correction. A systematic review of its clinical pharmacokinetics describes an elimination half-life of roughly one week, which is exactly why the drug is dosed weekly[3]; earlier pharmacokinetic work reached the same conclusion and attributed the long half-life to albumin binding and resistance to enzymatic degradation[4]. An accidental multiple of your dose is therefore an exposure that persists for days, not something that clears by morning.

Where small errors hide

  • Dead space. Fluid left in the needle hub after the plunger bottoms out is a recognized source of insulin dosing error[2]. On a 5-unit draw it is a large proportion of the dose; on a 50-unit draw it is negligible. The more concentrated your vial, the smaller your draw, and the more this matters.
  • Reading between marks. Standard U-100 syringes are often graduated in 2-unit increments. A dose that lands on 5 units or 68 units is being estimated, not measured.
  • Changing vials mid-titration. A new batch at a different concentration silently changes the correct number of units for the same prescribed milligrams. This is the most common way a stable patient's dose moves without anyone deciding it should.
  • Advice from other patients. A number of units posted online is only meaningful alongside that person's concentration, which is rarely stated. The unit figure travels; the concentration does not.
