Beyond-Use Dates on Compounded GLP-1s: Not an Expiration Date

The date on a compounded GLP-1 vial is a beyond-use date, and it is a different kind of claim from the expiration date on a manufactured medicine. An expiration date is supported by the manufacturer's own stability program on that exact product. A beyond-use date is assigned by the pharmacy that prepared your vial, under sterile compounding standards, and it is usually much shorter — because it is limited by microbiological risk as much as by chemistry[2].

Two different questions with one date on the label

A beyond-use date has to answer two questions at once. The chemical question is how long the drug remains potent in this formulation. The microbiological question is how long the preparation can be relied on to remain sterile. For sterile compounded preparations the second question usually dominates, and the criteria for setting the date reflect the conditions under which the preparation was made rather than the drug's intrinsic shelf life[3].

Why sterility drives the number

Guidelines on compounding sterile preparations set out the environmental controls, personnel practices and testing that a preparation's safety depends on[1]. The core logic is that sterility cannot be inspected into a finished vial — it is a consequence of how the product was made and how it is subsequently handled. That is why the date is tied to preparation conditions, and why entering the vial repeatedly with a needle shortens the period that applies to it.

This is the part patients most often miss. A vial can carry one date while unopened and a much shorter in-use period once first punctured. If your label states only one date, the in-use period is a separate question worth asking the pharmacy explicitly.

Why the chemistry is not a formality either

Degradation is not only a potency question. A 2026 analysis examined impurities and potential immunogenicity associated with follow-on and compounded GLP-1 receptor agonists[4] — that is, what related substances are present and what they might mean for immune responses. Degradation products are part of that picture, which is a reason to treat a beyond-use date as a limit to respect rather than a conservative guess to stretch.

A past date is not a judgment call

A beyond-use date is a boundary set by the people who made the preparation and know the conditions it was made under. Neither you nor we can evaluate whether a specific vial is still acceptable after that date — appearance in particular tells you almost nothing, since a solution can lose potency or carry contamination while looking completely normal. If a vial is past its date, the question goes to the pharmacy.

What to establish about your own vial

  1. The beyond-use date itself, and whether it assumes refrigeration.
  2. The in-use period after first puncture, which may be shorter than the date printed and is frequently not stated.
  3. The storage condition it depends on — a date is only valid under the conditions it was assigned for.
  4. Whether the preparation is single-dose or multi-dose, since that determines whether repeated entry was contemplated at all.
  5. What to do after a storage excursion, because leaving the vial out may affect the date even if the printed one has not arrived.

None of these are unreasonable things to ask, and a pharmacy that prepared the vial will have the answers. If a seller cannot supply them, that in itself is information about how the product is being handled.
