Insurance-first medical weight-loss clinic offering brand-name GLP-1s with physician and dietitian oversight. It bills insurance where it can and charges $299/mo self-pay when it cannot — a care fee only. Form does not sell the medication, so the drug is billed separately by the pharmacy and its cost is not published anywhere on the site.

  • Confirmed availability in all 50 states
  • Confirmed what the monthly price does and doesn't include
  • Checked the FDA warning-letter database for enforcement actions

What to expect when you sign up with Form Health

We walked Form Health’s public signup flow in July 2026 to document the process — the steps, pricing transparency, and what’s required — before you commit. (We don’t create accounts, enter medical information, or check out; this is the observable funnel, not a prescribing outcome.)

  1. 1Start with an online quiz via the Get Started / start-quiz flow.
  2. 2Receive care from an American Board of Obesity Medicine certified physician through the Form Health app.
  3. 3Attend frequent video visits with your clinical care team.
  4. 4If appropriate, FDA-approved obesity-management medication is prescribed.

Pricing

Prices re-verified

Self-pay program fee (medication billed separately)program fee
$299/mo

The ONLY price Form Health publishes anywhere, verbatim from its FAQ: 'If you do not have insurance or your insurance does not cover the Form Health program, we offer a self-pay plan for $299 per month that covers your Form clinician visits in addition to appointments with your Registered Dietitian, unlimited messaging and access to the Form platform. You are responsible for the cost of any lab work and medications.' This is a CARE fee, not a medication price — Form prescribes brand GLP-1s but does not sell them, so the drug is billed by the pharmacy on top and its cost is not published. HSA/FSA eligible. Insurance-first by design: the program is 'covered by most major private insurances and Medicare', and the $299 applies only when it is not.

Watch-outs

  • Higher brand-name pricing only
  • Premium pricing relative to market

Form Health is a real obesity-medicine clinic, not a medication vending machine

If most GLP-1 telehealth sites feel like a checkout page with a doctor attached, Form Health is the opposite. It is built to look and work like an actual obesity-medicine practice that happens to run over video: you get a clinician who specializes in weight, a Registered Dietitian, and unlimited messaging between visits. The trade-off is that it is also one of the more expensive ways to start, and it deliberately does not touch compounded medications. For the right person that focus is exactly the point; for a price-shopper it will feel like overkill.

How the money actually works: a care membership, not a med-included price

This is the part people get wrong, so read it twice. Form Health does not bundle the drug into one flat monthly number. Instead you pay a membership for the care team — your clinician visits, your dietitian appointments, and unlimited messaging — and the medication and any lab work are billed on top of that, separately. The membership runs roughly three hundred dollars a month when paid out of pocket.

The bigger lever is insurance. Form Health is designed to work with commercial insurance and will bill your plan for the care and, where possible, help you get a brand-name GLP-1 covered. If your plan covers the medication, your real out-of-pocket can land well below the cash sticker. If it does not, you are paying the membership AND the full pharmacy price of a brand drug, which is where the total gets steep.

  • What the membership covers: visits with your Form clinician, appointments with a Registered Dietitian, and unlimited messaging in between.
  • What it does NOT cover: the cost of the medication itself and any lab work — those are billed separately, to you or to your insurance.
  • Best case: insurance covers the brand GLP-1 and you mostly pay the care membership.
  • Worst case: no coverage, so you pay the membership plus a four-figure brand drug price each month.

Because that JSON-style flat brand rate is not actually how the live program prices, treat any single 'per month, all-in' number you see elsewhere with suspicion and confirm your specific cost — membership plus your insurance situation — before you commit.

Brand-name only: Wegovy, Ozempic, Mounjaro — no compounded shortcuts

Form Health prescribes FDA-approved, brand-name medications only. That means real-deal semaglutide (Wegovy, Ozempic) and tirzepatide (Mounjaro), dispensed through standard pharmacies — not compounded vials mixed by a 503A or 503B facility. There is no in-house compounding partner here at all.

This is a genuine philosophical fork in the GLP-1 market. Compounded-semaglutide sellers win on price; Form Health bets that you would rather have the exact molecule the large cardiovascular and weight trials studied, made by the original manufacturer, with no questions about dosing consistency or sourcing. If brand-name authenticity matters to you, that is a real differentiator and not just marketing.

The catch: brand-name pricing has no cheap floor

The flip side is unavoidable. Brand GLP-1s carry a list price north of a thousand dollars a month, and Form Health has no compounded option to fall back on if your insurance says no. There is no teaser first-month rate to soften the entry, either. If you are uninsured or your plan flatly excludes weight-loss drugs, this is a costly place to be.

The differentiator: a clinician plus a dietitian, on call

What you are really buying is the care model. Most budget telehealth gives you a brief async questionnaire and a refill. Form Health pairs you with a weight-focused clinician and a Registered Dietitian, and lets you message the team without limit between appointments. The medication is treated as one tool inside an ongoing treatment relationship, with nutrition coaching meant to protect against muscle loss and the rebound that hits people who stop a GLP-1 with no plan.

That is concierge-grade support, and it is the strongest reason to pick Form Health over a cheaper competitor. It is available in all 50 states, so the model is consistent wherever you live.

Who should choose it — and who should skip it

  • Choose it if: you have commercial insurance that might cover a brand GLP-1, you want a doctor-plus-dietitian relationship rather than a refill, and you specifically want brand-name, not compounded, medication.
  • Skip it if: you are paying fully out of pocket and price is the deciding factor, you are comfortable with compounded semaglutide, or you just want the lowest flat monthly number with the medication included.

Trust and oversight

On the safety side Form Health checks out well. It dispenses only FDA-approved brand medications through licensed pharmacies, builds the program around physician oversight and registered-dietitian support, and carries no FDA warning letters or enforcement actions on file. The main thing we could not lock down is the exact current pricing — our verification flagged a real discrepancy between an older flat-rate figure and the live membership-plus-insurance model — so price is the one detail to confirm directly with Form before enrolling. For how we weigh these factors, see our scoring methodology.

Bottom line

Form Health is one of the most serious, medically-grounded GLP-1 providers we have reviewed: brand-name drugs only, a clinician and a dietitian on your side, and an insurance-first billing model. It is not built to be cheap, and it does not pretend to be. If you have coverage and want real obesity medicine rather than a fast prescription, it earns its premium. If you are cash-paying and chasing the lowest number, a compounded provider will cost far less — just understand that is a different product.

Worth pricing against Sunlight ($88/month) before you commit — it's among the nearest alternatives to Form Health.

Form Health scores highest on state availability and ease of access.
