Do GLP-1s Cause Hair Loss? What All 5 Labels Say (2026)
Wegovy and Zepbound list hair loss in 3–6% of adults vs 1% on placebo; Ozempic, Rybelsus and Mounjaro give no rate. Why it happens and what helps.
Yes, GLP-1 drugs can come with hair loss, but mostly as a side effect of losing weight fast, not as damage to the hair itself. The two weight-management labels, Wegovy and Zepbound, list hair loss in about 3–6% of treated adults vs about 1% on placebo, and both say the hair loss was “associated with weight reduction”[3][5]. The diabetes labels (Ozempic, Rybelsus, Mounjaro) do not list it in their trial tables at all; alopecia shows up only in their post-approval reports, which carry no rate[1][2][4]. This page puts the five labels side by side, explains the shared mechanism once, and sends you to the page for your drug.
What each label says, side by side
| Drug (molecule, use) | Hair loss in the trial table? | Rate vs placebo | Read more |
|---|---|---|---|
| Ozempic (semaglutide injection, type 2 diabetes) | No. Alopecia is listed under postmarketing reports only | No rate given | Ozempic page |
| Rybelsus / Ozempic tablets (oral semaglutide, type 2 diabetes) | No. Alopecia is listed under postmarketing reports only | No rate given | Semaglutide page |
| Wegovy (semaglutide 2.4 mg or 7.2 mg weekly injection, or 25 mg daily tablet, weight management) | Yes | 2.4 mg: 3% vs 1%. 7.2 mg: 6% vs 1%. Adolescents on 2.4 mg: 4% vs 0% | Wegovy page |
| Mounjaro (tirzepatide injection, type 2 diabetes) | No. Alopecia is listed under postmarketing reports only | No rate given | Mounjaro page |
| Zepbound (tirzepatide injection, weight management) | Yes | 5 mg: 5%. 10 mg: 4%. 15 mg: 5%. Placebo: 1% | Zepbound page |
Two patterns stand out. First, hair loss is a trial-table finding only on the labels written for weight loss, the indication where people lose the most weight. A diabetes label that leaves it off the table is not evidence that the drug is safer for hair; its trials simply did not record hair loss often enough to make the table. Second, women report it far more than men. On Wegovy, a pool of three trials found hair loss in 4% of women vs 0.9% of men on 2.4 mg, and 8.4% of women vs 0.2% of men on 7.2 mg[3]. On Zepbound it was 7.1% of women vs 0.5% of men[5]. No Zepbound-treated patient in those trials stopped the drug because of hair loss[5]. The Wegovy tablet label gives no separate hair-loss figure; it says common side effects were similar to the injection's[3].
What research outside the labels adds
- Across trials, the risk is roughly three times placebo. A 2026 meta-analysis of 9 interventional studies (7 randomized) covering 4,114 GLP-1 users found a risk ratio of 3.25 (95% CI 1.44–7.36), and 3.59 when limited to obesity trials. The pooled rate of hair loss on treatment was 3.9%[6].
- In everyday diabetes care, the signal is smaller but present. A BMJ target-trial emulation in Penn Medicine records followed about 12,000 people with type 2 diabetes starting a GLP-1. Their risk of a hair-loss diagnosis was higher than with SGLT-2 inhibitors (hazard ratio 1.37) or DPP-4 inhibitors (1.68), limited to non-scarring hair loss. The authors note the absolute risk is low[7].
- Causality is still open. A 2026 dermatology review concluded the evidence “is not conclusive, causality has not been established,” and that the most common patterns are telogen effluvium and a worsening of existing pattern hair loss[8]. That review was funded by a hair-care company, so read its management advice with that in mind.
Why it happens (the short version)
Most of the time this is telogen effluvium: a big physical stress pushes many growing hairs into their resting phase at once, and they fall out together weeks later. Crash dieting, low protein intake, iron deficiency and hypothyroidism are all on the standard trigger list[9], and losing a large share of body weight in months is that kind of stress. Headington's 1993 framework explains how a single trigger can synchronize the cycle across the scalp[10]. The shedding usually starts about 3 months after the drug is started[8], which is why it often shows up around the time you reach a higher dose and is easy to blame on the dose.
For the week-by-week time course, see how long GLP-1 side effects last.
What helps
- Eat enough protein and nutrients even when you aren't hungry. Low protein intake is itself a shedding trigger[9], and reviewers advise adequate protein and micronutrients for anyone on these drugs[8].
- Ask for basic labs if shedding is heavy. Iron deficiency and an underactive thyroid can cause the same shedding[9]. A ferritin test is the usual starting point; our GLP-1 and iron deficiency page covers what the numbers mean.
- Don't stop the drug on your own. In telogen effluvium, shedding stops once the weight loss stops[8]. Talk with your prescriber first: slower dose increases or a dose change are options they can weigh.
- Treat it if it hangs on. If shedding lasts or you see pattern thinning, minoxidil and finasteride have the best evidence. See finasteride and minoxidil on a GLP-1 for doses and trial data.
When it is probably not a GLP-1 shed: see a dermatologist
- Round bald patches, or loss of eyebrows or body hair (possible alopecia areata, which is autoimmune)[8].
- A scalp that is red, scaly, itchy, painful or scarred.
- Shedding with fatigue, weight change you can't explain, or feeling cold all the time (check thyroid and iron)[9].
- Shedding that keeps going well after your weight has leveled off.
