Ozempic at Costco

Novo Nordisk’s self-pay offer is not a Costco exclusive. Novo says the same pricing reaches “over 70,000 pharmacy locations across the US,” and it names Costco as one of several participating organizations, alongside GoodRx, WeightWatchers, Ro, LifeMD and eMed.

Zepbound at Costco

Zepbound — Costco does carry tirzepatide. Its price tool lists Zepbound as a single-dose auto-injector pen in all six strengths — 2.5, 5, 7.5, 10, 12.5 and 15 mg — sold four pens to a month, at a member cash price rather than Lilly’s self-pay price.

Lilly’s own self-pay prices are lower and sit elsewhere: $299 a month for 2.5 mg and $399 for 5 mg, then $449 for 7.5 mg through 15 mg — but that $449 holds only if the prescription is refilled within 45 days, and Lilly lists the regular price as $499 for 7.5 mg and $699 for 10, 12.5 and 15 mg. Those prices are for the KwikPen and the single-dose vial through LillyDirect; the vial is, in Lilly’s words, “Exclusively available through LillyDirect,” with retail pick-up at Walmart rather than Costco.

Mounjaro at Costco

Mounjaro — Mounjaro is stocked in every strength from 2.5 mg to 15 mg, four pens to a box, and the tool shows a member cash price with no discount program attached.

Mounjaro is approved for type 2 diabetes, and its manufacturer savings card is an insurance copay card, so a self-pay Mounjaro fill at Costco lands close to list price. A cash-paying patient who wants tirzepatide for weight management is usually looking at Zepbound through LillyDirect instead.

How to buy a GLP-1 at Costco

  1. Get a prescription. Any US prescriber can send one, and Costco Pharmacy also lists virtual doctor visits among its member benefits.
  2. Ask your prescriber to send it to your local Costco Pharmacy, or move an existing prescription over at rx.costco.com.
  3. Tell the pharmacy you are paying self-pay and ask for the Member Value Pricing figure. Do not let the claim go through insurance first — a claim billed to Medicare, Medicaid or a private plan is not eligible for the program price.
  4. Choose warehouse pick-up or home delivery. Costco’s tool shows the same price for both on every GLP-1 we checked, and notes that shipping and handling are not included.
  5. For Zepbound at Lilly’s self-pay price, enroll with LillyDirect first and expect delivery or Walmart pick-up — Costco’s shelf price for Zepbound is the full member price.
Costco Member Value Pricing is administered by Costco Pharmacy with Ventegra and, in Costco’s own words, “is not insurance or intended as a replacement for insurance.” The $199 introductory price covers two fills of the 0.25 mg and 0.5 mg doses and rises to $349 after December 31, 2026.
