Dulaglutide in plain English
Dulaglutide, sold as Trulicity, is a once-weekly GLP-1 receptor agonist for type 2 diabetes. Its label covers two things: improving glycemic control in adults and children aged 10 and older, and reducing the risk of major adverse cardiovascular events in adults with type 2 diabetes who have established cardiovascular disease or multiple cardiovascular risk factors.
The dose ladder starts at 0.75 mg once weekly, may rise to 1.5 mg after four weeks, and can climb in 1.5 mg steps after at least four weeks at each level, to a maximum of 4.5 mg once weekly. The pen is widely described as the easiest in the category to use, which matters more than it sounds — device friction is a real cause of missed doses.
Its evidence base rests on REWIND, which followed 9,901 adults for a median 5.4 years and found the cardiovascular composite in 12.0% on dulaglutide versus 13.4% on placebo (hazard ratio 0.88). What makes REWIND distinctive is that most participants had not already had a cardiovascular event, so the benefit was shown in a primary-prevention population. The tolerability cost was substantial: 47.4% reported a gastrointestinal adverse event over follow-up versus 34.1% on placebo.
Here is the confusion that matters for readers of a weight-loss site. Dulaglutide is not FDA-approved for chronic weight management. Its label is a diabetes label with a cardiovascular claim attached. People do lose some weight on it, but less than on the agents developed for that purpose, and prescribing it for weight alone is off-label.
Worth asking your clinician: whether your goal is primarily glucose control or primarily weight, since that should decide the drug; whether a weight-indicated agent is covered for you; and how to handle the gastrointestinal side effects that made a third more people uncomfortable in REWIND.
