Does Semaglutide Help You Live Longer? Longevity, Mortality and Aging Evidence

In randomized trials, deaths from any cause were about 20% lower on semaglutide in people with heart or kidney disease. What SELECT, FLOW and SOUL show, the early aging-clock and mouse lifespan data, and where longevity hype goes too far.

Is semaglutide a longevity drug? Large randomized trials point to fewer deaths from any cause. In an analysis of the SELECT trial, adults with heart disease and obesity who took Wegovy's 2.4 mg dose had a 19% lower rate of death from any cause than those on placebo,[1] and in FLOW, people with diabetes and kidney disease had 20% fewer deaths.[2] Early studies hint that it may also slow measures of biological aging,[5] and in 2026 it extended lifespan in older female mice.[7] But no trial has shown that semaglutide slows aging itself in healthy people. Here is what the evidence supports, and where the “longevity” hype goes beyond it.

About this article

The mortality results come from large randomized trials in people with heart disease, kidney disease or diabetes, not in healthy adults. The aging-clock and lifespan findings come from a small post hoc analysis and a mouse study. Semaglutide is not FDA-approved to extend life or slow aging. For other drugs and supplements marketed for longevity, see our metabolic longevity overview. This is not medical advice.

The short answer

  • Fewer deaths, in high-risk people. SELECT: all-cause death HR 0.81 (95% CI 0.71–0.93) in adults with cardiovascular disease and overweight or obesity.[1] FLOW: HR 0.80 (0.67–0.95) in type 2 diabetes with chronic kidney disease.[2]
  • Not only heart deaths. In SELECT, non-cardiovascular deaths also fell (HR 0.77), mainly fewer deaths from infection.[1]
  • Class-wide signal. Across 21 trials of GLP-1 drugs (99,599 patients), all-cause death was 12% lower (IRR 0.88).[4]
  • Biological aging: early, small, unproven. One 32-week post hoc analysis in 84 people with HIV found slower epigenetic aging clocks on semaglutide.[5] A mouse study found longer lifespan.[7] Neither shows that semaglutide slows aging in healthy humans.

What the trials show about death from any cause

Semaglutide and all-cause mortality in large randomized trials
TrialWhoAll-cause deathNotes
SELECT (injectable 2.4 mg)17,604 adults with cardiovascular disease and overweight or obesity, no diabetesHR 0.81 (0.71–0.93)Non-CV death HR 0.77; CV death HR 0.85
FLOW (injectable)3,533 adults with type 2 diabetes and chronic kidney diseaseHR 0.80 (0.67–0.95)CV death HR 0.71
SOUL (oral 14 mg)9,650 adults 50+ with type 2 diabetes and heart or kidney diseaseNot reported in the abstractMajor cardiovascular events HR 0.86

SELECT randomized 17,604 adults[10] and recorded 833 deaths. Compared with placebo, semaglutide lowered death from any cause (HR 0.81), cardiovascular death (HR 0.85, 95% CI 0.71–1.01) and non-cardiovascular death (HR 0.77). “Infection was the most common cause of non-CV death and occurred at a lower rate in the semaglutide vs the placebo group” (62 vs 87). Semaglutide didn't prevent COVID-19, but fewer people on it died of COVID-19 (43 vs 65).[1] The trial ran partly during the pandemic, which shaped the cause-of-death mix.

How strong is the SELECT result? The trial's main finding was for combined heart attacks, strokes and cardiovascular deaths;[10] the all-cause death figure comes from a later analysis of the trial's deaths, and cardiovascular death on its own just missed statistical significance (HR 0.85, 95% CI 0.71–1.01).[1] So it supports, rather than proves, a survival benefit. In FLOW, death from any cause was one of the trial's confirmatory secondary outcomes, and it was 20% lower with semaglutide.[2] In SOUL, oral semaglutide reduced major cardiovascular events (HR 0.86), but its confirmatory secondary outcomes “did not differ significantly between the two groups.”[3] So the mortality evidence is clearest for the injectable drug in SELECT and FLOW. For the trials themselves, see the SELECT trial and the FLOW trial.

Looking across the drug class, a 2025 JACC meta-analysis of 21 trials found “conclusive, high-certainty evidence” that GLP-1 drugs reduced all-cause death (IRR 0.88; number needed to treat 121).[4]

How many years of life might that add?

A UK modeling study applied SELECT's results to 19,117 people in health records who met SELECT's eligibility criteria. It estimated a 1.9-year gain in life expectancy and 2.0 more years free of cardiovascular events, with larger gains in younger people.[9] That is a projection, not something measured in a trial, and it applies only to people like those in SELECT: aged 45 or older, with established heart disease, a BMI of 27 or more and no type 2 diabetes.

Does semaglutide slow biological aging?

“Living longer” and “aging more slowly” are different questions. Epigenetic clocks estimate biological age from patterns of DNA methylation in the blood. A 2026 Nature Communications analysis looked at these clocks in a 32-week randomized trial of semaglutide in 84 adults with HIV-associated lipohypertrophy, a condition of abnormal fat distribution. Semaglutide reduced aging on several clocks, for example PhenoAge by −4.9 years per year and DunedinPACE by 9%.[5]

The authors list the limits plainly: epigenetic aging “was not pre-specified,” and “the post hoc design, modest sample size, HIV-specific cohort, and 32-week follow-up limit generalizability.”[5] A separate single-arm study in 41 people with HIV and fatty liver found no consistent change in the clocks over 24 weeks, though people whose DunedinPACE fell lost more liver fat.[6] Whether these clocks predict how long people actually live on a GLP-1 is not known.

The 2026 mouse lifespan study

In a 2026 Nature study, giving semaglutide to 20-month-old female mice “improved physiological function, attenuated hallmarks of ageing” and, with continued treatment, extended lifespan. It reproduced many effects of calorie restriction, with some benefits beyond it.[7] It suggests GLP-1 activation can act on aging biology, in mice. But it was in female mice only, and results in mice don't always carry over to people. Compare the history of rapamycin.

Healthspan: frailty and older adults

In a SELECT analysis by frailty, the benefits of semaglutide on heart events and all-cause death held across frailty levels. People on semaglutide were more likely to move to a less frail category over two years (odds ratio 2.46) and less likely to become frailer (0.47).[8] For older adults, the main caution is muscle loss with rapid weight loss. See GLP-1s after 65 and muscle.

Should healthy people take semaglutide for longevity?

The evidence doesn't support it. The mortality benefit was shown in people at high risk: established heart disease with overweight or obesity (SELECT), or diabetes with kidney disease (FLOW).[1][2] Nobody has tested whether semaglutide extends life in lean, healthy adults, and the drug carries real side effects, cost and the expectation of long-term use. If you have obesity plus heart disease, kidney disease or diabetes, the case for treatment already rests on outcomes, not longevity marketing. For a related drug, see tirzepatide and longevity. For supplements sold for aging, see NAD+ and aging in human trials.

Bottom line

Semaglutide has some of the strongest survival evidence of any weight-loss medicine: in large randomized trials, deaths from any cause were about 20% lower in people with heart disease, kidney disease or diabetes, a confirmed outcome in FLOW and a supportive analysis in SELECT. Evidence that it slows biological aging is early: one small post hoc human analysis and one mouse study. It is a treatment for disease that may help people live longer, not a proven anti-aging drug for healthy people.

Frequently Asked Questions

In people at high risk, yes. In the SELECT trial of adults with heart disease and overweight or obesity, Wegovy's semaglutide lowered death from any cause by 19 percent (hazard ratio 0.81). In the FLOW trial of people with diabetes and kidney disease, deaths were 20 percent lower. It has not been tested for longevity in healthy people.
It is not proven. One small 2026 analysis of a 32-week trial in people with HIV found slower epigenetic aging clocks with semaglutide, and a 2026 study in female mice found longer lifespan. Neither shows that semaglutide slows aging in healthy humans, and the human analysis was not planned in advance.
A UK modeling study applying SELECT results to people with heart disease and overweight or obesity estimated about 1.9 extra years of life expectancy, more in younger people. This is a projection from trial data, not a measured result, and applies only to people similar to those in SELECT.
The evidence does not support it. The mortality benefits were seen in people with established heart disease, kidney disease or diabetes. There are no trials in healthy adults, and semaglutide has side effects, costs and usually requires long-term use.
