What “Ozempic pen clicks” actually means

Ozempic’s pen is a dial-a-dose injector: turning the dose selector produces an audible click, and people searching for a “click chart” are usually trying to translate that clicking into a dose. The Ozempic Instructions for Use answers this directly — and its answer is not a click-to-milligram table. It says the opposite [2]:

“You will hear a ‘click’ every time you turn the dose selector. Do not set the dose by counting the number of clicks you hear.” And separately: “The dose selector clicks differently when turned forward or backward. Do not count the pen clicks.”

Instead, the label tells patients to watch the dose counter window and stop turning the selector once it shows the prescribed number, confirmed against the dose pointer [2].

Why this page does not publish a clicks-to-mg chart

Beyond the explicit instruction not to count clicks, the label describes each pen’s dial as having a fixed set of selectable stops — not a continuously variable dial:

• 2 mg pen: “Only doses of 0.25 mg or 0.5 mg can be selected with the dose selector.”
• 4 mg pen: “Only doses of 1 mg can be selected with the dose selector.”
• 8 mg pen: “Only doses of 2 mg can be selected with the dose selector.”

The FDA prescribing information never publishes a click-to-milligram ratio for any Ozempic pen [2], and the mechanism it describes does not support one: the selector is built to stop only at the printed, labeled dose for that pen, with no intermediate detent to count toward. A third-party number claiming otherwise is not sourced from the label. Where the label does support arithmetic — how many full doses one pen contains — that math is shown transparently on the “Doses per pen” tab above, and every row is marked as either a direct label quote or derived math we performed.

How the dosage schedule works

Per the FDA label [1], Ozempic starts at 0.25 mg once weekly for 4 weeks — this starting dose is not intended to improve glycemic control on its own. After 4 weeks, the dose increases to 0.5 mg once weekly, which is often the maintenance dose. Unlike Wegovy, Mounjaro, or Zepbound, Ozempic’s further increases are conditional, not automatic: if additional glycemic control is needed, the dose may increase to 1 mg after at least 4 weeks at 0.5 mg, and then to 2 mg (the maximum) after at least 4 weeks at 1 mg. To reduce the risk of sustained eGFR decline, end-stage kidney disease, and cardiovascular death in patients with chronic kidney disease, the label separately directs an increase to 1 mg once weekly after at least 4 weeks at 0.5 mg [1].

Ozempic is FDA-approved as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes, to reduce the risk of major adverse cardiovascular events in adults with type 2 diabetes and established cardiovascular disease [1][3], and to reduce the risk of sustained eGFR decline, end-stage kidney disease, and cardiovascular death in adults with type 2 diabetes and chronic kidney disease [1]. It is not FDA-approved for chronic weight management — semaglutide at the higher, weight-management dose and titration is approved separately under the brand name Wegovy.

Storage

Per the FDA prescribing information [1]: before first use, store Ozempic in the refrigerator at 36°F to 46°F (2°C to 8°C) and do not freeze. After first use, the pen may be kept at room temperature (59°F to 86°F) or refrigerated for up to 56 days. Keep the pen cap on when not in use, and do not use Ozempic if it has been frozen.
