Epitalon (epithalon) peptide
Also known as Epithalon, Epithalone, AEDG peptide, Ala-Glu-Asp-Gly
Epitalon (epithalon) is a four-amino-acid peptide modeled on a pineal-gland extract and marketed for telomeres and longevity. Nearly all of the evidence comes from one Russian research group, and no independent human trial exists.

Overview
Epitalon (also spelled epithalon or epithalone) is a synthetic peptide of four amino acids: alanine, glutamic acid, aspartic acid and glycine, often written AEDG. It was designed in Russia as a synthetic stand-in for epithalamin, an extract of the bovine pineal gland [6]. It is marketed for longevity, with claims that it switches on telomerase and lengthens telomeres.
Those claims rest on a small literature that comes almost entirely from Vladimir Khavinson's group in St. Petersburg. The telomerase finding is a single 2003 cell-culture study [2]. The human mortality reports used the pineal extract, not synthetic epitalon [4] [5]. An independent 2025 review calls the compound bioactive and promising while saying its mechanisms remain uncertain [6]. Under a draft HHS enforcement policy announced October 28, 2026, HHS says it does not intend to take action against 503A pharmacies and physicians that compound epitalon under ten conditions, by injection in intermittent courses only [1]. That changes how it can be obtained in the US, not what is known about it.
Regulatory status
Epitalon is not FDA-approved for any use. On October 28, 2026, HHS announced a draft, interim enforcement-discretion policy, which is not final and not binding on HHS or the public (comments are open until November 13, 2026): while FDA decides through rulemaking whether epitalon belongs on the 503A bulk drug substances list, HHS does not intend to take action against a State-licensed pharmacy, Federal facility, or licensed physician that compounds epitalon acetate when ten conditions are all met [1]. The policy states that compounded drugs are not FDA-approved and that FDA does not review their safety, effectiveness, or quality.
Route limits: the policy covers epitalon acetate in subcutaneous or intramuscular injectable forms dispensed in intermittent courses [1]. Continuous use, oral products, and nasal products fall outside it. The policy does not define a course length or a dose.
The other conditions include a patient-specific prescription; bulk peptide from an FDA-registered manufacturer with a certificate of analysis covering identity, purity, potency, endotoxin, heavy metals, microbial limits and impurities; a single active ingredient per product (no epitalon blends); the label statement "This is a compounded drug product. Not for resale."; a 510(k)-cleared syringe as the only co-packaged device; and reporting of serious adverse events and quality problems to FDA MedWatch and the states [1].
History: epitalon was in FDA's Category 2 from September 29, 2023 until April 22, 2026. CDER recommended against listing it and the Pharmacy Compounding Advisory Committee voted in favor [1]. HHS gave three reasons for including it: newer certificates of analysis that answer CDER's quality questions, documented compounding use, and its reading that CDER's safety concerns were mechanistic rather than based on observed harm, noting that the limited controlled human exposure reported no side effects and that documented regimens are intermittent [1]. An independent analysis of the advisory committee review appears in the compounding literature [8].

How it works
The central claim is about telomeres, the protective caps on chromosomes that shorten as cells divide. In a 2003 study, adding epitalon to cultured human fetal fibroblasts that normally lack telomerase induced expression of the enzyme's catalytic subunit, telomerase activity, and telomere elongation [2]. That is one cell-culture experiment from the originating group, and it has not built into an independent replication literature since.
The 2025 independent review lists other reported effects from cell, animal and computer-modeling work: influence on melatonin synthesis, changes in interleukin-2 mRNA, and effects on several enzymes including telomerase. Its authors write that it remains uncertain whether these are the sole mechanisms of action, and that physico-chemical and structural studies of the peptide are still limited [6]. A caution that applies to any telomerase claim: switching the enzyme on broadly is not automatically good, because telomerase reactivation is also a feature of many cancers.
What the evidence says
Synthetic epitalon in people: the main published human report is a 2002 paper on retinitis pigmentosa, an inherited retinal disease. It describes epitalon improving retinal function in Campbell rats and reports a positive clinical effect in 90% of treated patients with degenerative retinal lesions [3]. The abstract does not give the number of patients or describe a control group.
The pineal extract in people: a 2003 report followed 266 older adults for 6 to 8 years after courses of the thymus extract thymalin, the pineal extract epithalamin, or both, given during the first 2 to 3 years. It reported a mortality rate 1.6 to 1.8 times lower in the epithalamin group than in controls, and 2.5 times lower with thymalin plus epithalamin [4]. A 2011 randomized study gave 39 older coronary patients six courses of epithalamin over three years alongside standard treatment, compared with 40 on standard treatment alone, and reported slower cardiovascular aging and significantly lower mortality over 15 years of follow-up [5]. Both studies come from the same research network, both used the extract rather than synthetic epitalon, and neither has been repeated by an independent group.
Evidence grade: D. The positive data are concentrated in one group, published largely in Russian and specialty journals, and the strongest human numbers belong to a different product. No independent randomized trial of synthetic epitalon for aging, lifespan, or any other outcome exists in PubMed, and no study reports weight loss.
Typical dosing
There is no established human dose of synthetic epitalon. The abstracts above report courses of treatment but do not state a dose for the synthetic peptide, and the extract studies used a different product. The HHS policy limits compounded epitalon to injection in intermittent courses but does not specify a dose or a course length [1]. We do not publish a protocol. If a clinician prescribes compounded epitalon, the dose and the length and spacing of courses are their decision, not a validated regimen.
Safety & side effects
Long-term human safety is unknown. HHS's justification states that the limited controlled human exposure reported no side effects [1], but that exposure is small and comes largely from the originating group. Absence of reported side effects in a few small studies is not the same as an established safety record, and the telomerase claim adds a specific question, because telomerase activity is a hallmark of many cancers.
Quality is a documented problem in the unregulated market: Belgian regulatory chemists identified epitalon in two illegal pharmaceutical preparations promoted for cancer, old age and retinitis pigmentosa [7]. A product compounded under the HHS policy must instead come from a registered manufacturer with a lot-level certificate of analysis and contain epitalon alone [1]. That improves what is in the vial. It does not supply the missing evidence of benefit.
Frequently asked questions
Is epitalon FDA-approved?
No. Epitalon has no FDA approval for any use. Under a draft policy announced October 28, 2026, HHS has said it will not take enforcement action against 503A pharmacies and physicians that compound epitalon acetate by subcutaneous or intramuscular injection in intermittent courses, under ten conditions. That is enforcement discretion while FDA works on the 503A bulks list, not approval.
Does epitalon lengthen telomeres in people?
That has not been shown. The telomere finding is a 2003 study in cultured human fetal cells from the group that developed epitalon. No independent human study has shown that epitalon lengthens telomeres or slows aging.
What about the Russian studies showing lower mortality?
Those reports used epithalamin, a pineal-gland extract, not synthetic epitalon. One followed 266 older adults and reported 1.6 to 1.8 times lower mortality with the extract; another randomized 79 coronary patients and reported lower mortality over 15 years. Both come from the same research network and have not been replicated independently.
What does 'intermittent courses' mean in the HHS policy?
The policy covers epitalon only when it is dispensed in intermittent courses rather than for continuous use. It does not define the length of a course or a dose. HHS cited the intermittent pattern of documented regimens as one reason for including it.
Does epitalon help with weight loss?
No study reports weight loss from epitalon in people, and its proposed mechanisms have nothing to do with appetite or fat loss. If weight is the goal, FDA-approved GLP-1 medications have large randomized trials behind them.
Is Epitalon banned in sports?
Epitalon is not named on WADA's 2026 Prohibited List. Its S0 class prohibits at all times any pharmacological substance not covered elsewhere on the List and not approved by any governmental health authority for human therapeutic use, so an athlete subject to anti-doping rules should check with their anti-doping organization before using it.
