Missed dose

A missed dose is the most common practical question on a GLP-1, and the answer is different for every product. The rules come straight from the labels, and guessing is the wrong approach because the windows do not match.

Zepbound and Mounjaro allow a missed weekly injection to be taken within four days (96 hours); past that, skip it and resume on the regular day, keeping at least 72 hours between two doses. Ozempic gives five days. Wegovy injection works from the other direction: take the missed dose if the next scheduled one is more than two days (48 hours) away, and skip it if the next dose is less than two days out. Saxenda, being daily, says simply resume the next day and never double up. Rybelsus, Ozempic tablets and Wegovy tablets all say the same: skip it, take the next one tomorrow.

One rule matters more than the others because it affects your dosing plan rather than just one week. The Wegovy label directs that if two or more consecutive injections are missed, escalation should be reinitiated at a lower dose to reduce the risk of gastrointestinal adverse reactions. Saxenda has a similar instruction: after three or more days off, restart at 0.6 mg and climb again. You cannot simply pick up where you left off after a long gap.

The confusion that causes real harm is doubling up. No label permits it, and taking two doses close together is a straightforward way to produce severe nausea and vomiting. The other confusion is assuming the weekly drugs are interchangeable on this — four days, five days and two days are three different rules.
