How the missed dose rules differ by drug
Each GLP-1 receptor agonist has a specific missed-dose protocol in its FDA prescribing information. The critical variable is the window — how many days after the scheduled dose you can still take it:
- Wegovy (semaglutide 2.4 mg) [1]: If the missed dose is within 5 days of the scheduled day, take it as soon as possible. If more than 5 days have passed, skip the missed dose and take the next dose on the regularly scheduled day. If 2 or more consecutive doses are missed, consider restarting at a lower dose to mitigate GI side effects — contact your prescriber.
- Ozempic (semaglutide 0.5/1/2 mg) [2]: Same 5-day window as Wegovy. Within 5 days, take the missed dose. If more than 5 days, skip and resume the regular schedule.
- Zepbound (tirzepatide) [3]: If the missed dose is within 4 days (96 hours), take it as soon as possible. If more than 4 days have passed, skip and take the next dose on the regular schedule. Subsequent doses should be administered at least 3 days (72 hours) apart.
- Mounjaro (tirzepatide) [4]: Same 4-day (96-hour) window as Zepbound. Within 4 days, take the missed dose. If more than 4 days, skip. Subsequent doses at least 3 days apart.
- Foundayo (orforglipron) [5]: This is a daily oral GLP-1, not a weekly injectable. If you miss a dose, skip it and take the next dose at your regularly scheduled time. Do not take two doses in one day.
Why the windows differ
Semaglutide has a longer elimination half-life (~7 days) compared to tirzepatide (~5 days), which is why semaglutide-based drugs (Wegovy, Ozempic) have a 5-day make-up window while tirzepatide-based drugs (Zepbound, Mounjaro) have a 4-day window. Taking a late dose too close to the next scheduled dose would result in overlapping peak plasma concentrations and increased side effects.
Orforglipron (Foundayo) is a daily oral GLP-1 with a much shorter half-life (~25-35 hours), so the guidance is simply to skip and take the next day’s dose at the regular time.
