Best TB-500 Providers in 2026 — Ranked & Reviewed
TB-500 is a seven-amino-acid fragment associated with thymosin beta-4, a protein involved in cell migration and wound healing, and is often discussed alongside BPC-157 for recovery. The human trials that exist used the full pharmaceutical protein, not the fragment. In a draft enforcement policy dated October 28, 2026, HHS says it does not intend to bring enforcement action against licensed 503A pharmacies and physicians that compound TB-500 acetate for subcutaneous or intramuscular injection, if ten conditions are met. That is enforcement discretion, not FDA approval. See below for the current status.

Status as of October 30, 2026
Under a draft HHS policy announced October 28, 2026, HHS says it will not take enforcement action against 503A pharmacies and physicians compounding TB-500 under ten conditions. It is not FDA approval, and the policy is not final or binding; public comments are open until November 13, 2026.
- Forms the policy covers
- Injection (under the skin or into muscle) only. Any other form is labeled as not covered.
- Left off this list
- Blends with BPC-157 or other peptides.
- Key conditions
- A patient-specific prescription, a single-ingredient product, a certificate of analysis from an FDA-registered manufacturer, and MedWatch reporting (ten conditions in all).
- What a listing means
- The clinic sells single-ingredient TB-500 on its own site (checked October 30, 2026). We have not confirmed its pharmacy meets the policy's other conditions. Ask whether a 503A pharmacy (not a 503B outsourcing facility) fills your order, and in what device.
- TB-500 sat on FDA's Category 2 list between September 29, 2023 and April 22, 2026. It is not FDA-approved, and it remains banned in WADA-governed sports.

Detailed Reviews
A note on prices below: any dollar figure in these provider summaries refers to that clinic’s GLP‑1 weight-loss program, not to TB-500. The TB-500 price is the one beside each clinic in the comparison table above, read from that clinic’s own product page. Where it reads “Check pricing”, the clinic prescribes TB-500 but publishes no price for it — you are quoted after the consultation.
Pepti
Pepti is a Delaware-registered telehealth platform selling doctor-prescribed compounded peptides and GLP-1s, with a published shipping list of 49 states plus DC. Injectable semaglutide starts at $99 and tirzepatide at $159 per 28-day supply, rising with dose to $199 and $319. Each price covers medication, physician review, labs, refill management and shipping, with no membership fee. Pepti is a platform, not a prescriber, and does not bill insurance.
Score Breakdown
Pros
- ✓Injectable semaglutide starts at $99 and tirzepatide at $159 all-in, with no membership fee on top.
- ✓Publishes the exact price for every dose of both drugs, so you can see what titrating up will cost before you start.
- ✓Publishes a shipping list of 49 states plus DC, and a provider denial means a full refund.
- ✓Names the reviewing physician, Dr. Gene Lee MD, and posts third-party sterility and potency results by lot.
Cons
- ✗Billed every 28 days rather than monthly, so you pay 13 times a year, not 12.
- ✗Price rises with dose: semaglutide tops out at $199 and tirzepatide at $319 per supply.
- ✗The site says it serves 50 states, but its own list leaves out Oklahoma and counts DC instead.
- ✗No compounding pharmacy is named in advance; you only learn it from your medication label.
- ✗The listed address, a Newark Delaware suite, is a shared registered-agent address rather than a clinical office.
- ✗Cash-pay only with no insurance billing, and medication can't be refunded once it ships.
Live Vital
Verified partnerLive Vital is a physician-led telehealth practice founded by Dr. Patrick Taylor, MD, offering compounded GLP-1 weight loss (semaglutide advertised from $99/mo and tirzepatide from $183/mo, both sold as 10-week protocols) alongside peptide and hormone therapy. Every plan is doctor-prescribed, filled by a licensed US pharmacy, and shipped to your door, with unlimited care-team messaging. Fully telehealth in all 50 states.
Score Breakdown

- ✗Compounding pharmacy isn't named (only 'a licensed US pharmacy')
- ✗Medications and lab work are generally non-refundable once ordered, and consultation refunds are at the practice's discretion

How we rank & what counts as “legit”
Every provider in this ranking is scored against our published six-factor rubric[1] — value, effectiveness, user experience, trust & safety, accessibility, and support.
Brand-name Wegovy, Zepbound, Ozempic, and Mounjaro are separately FDA-approved under their own NDA numbers[4][5]. Published Phase 3 efficacy for semaglutide 2.4 mg (~14.9% mean weight loss over 68 weeks) comes from the STEP 1 trial[6], and for tirzepatide (~20.9% at the 15 mg dose over 72 weeks) from SURMOUNT-1[7]; the SURMOUNT-5 head-to-head published in 2025 compared the two directly[8].
Insurance coverage for anti-obesity medications varies widely by state Medicaid program and commercial plan[9][10]. Compounded and brand-name GLP-1s are generally FSA/HSA eligible with a prescription under IRS Publication 502[11].
