Specialty pharmacy

A specialty pharmacy dispenses high-cost, complex medications that need special handling — refrigeration, cold-chain shipping, clinical monitoring, or patient training on an injection device. GLP-1 pens tick several of those boxes, so many commercial plans route Wegovy, Zepbound and Mounjaro through a contracted specialty pharmacy rather than letting you pick them up at a retail counter.

The channel has genuine advantages. Specialty pharmacies employ pharmacists who know these drugs, they often handle prior-authorization paperwork on your behalf, they ship in validated cold packaging, and they call to arrange refills. For a medication that costs hundreds of dollars a fill and spoils if it sits on a porch in July, that is not trivial.

The disadvantage is loss of choice, and it is worth understanding why it happens. The FTC’s 2024 interim staff report on pharmacy benefit managers found that the largest PBMs are vertically integrated with retail and specialty pharmacies, and that pharmacies affiliated with the three largest PBMs account for nearly 70% of all specialty drug revenue. So when a plan requires a specific specialty pharmacy, that pharmacy is frequently owned by the same corporate parent as the PBM writing the rule.

Practical friction follows. Shipments must be scheduled around travel and weather. A pen that arrives warm needs to be reported immediately, not used. Transferring a prescription between pharmacies mid-year can restart an authorization. And a specialty pharmacy that is out of stock cannot always be substituted with a local one if the plan will not cover it there.

Worth asking: which pharmacy your plan requires, whether they ship with temperature monitoring, what to do if a shipment arrives warm or late, and whether an exception is possible if you cannot reliably receive deliveries.
