Which has better weight loss data, Zepbound or Foundayo?

Zepbound, by roughly a 2x margin. SURMOUNT-1 (Jastreboff 2022 NEJM, PMID 35658024) showed tirzepatide 15 mg cut body weight 20.9% at 72 weeks in adults with obesity. ATTAIN-1 (Wharton 2025 NEJM, PMID 40960239) showed orforglipron 17.2 mg cut body weight 11.2% at 72 weeks — also a real, clinically meaningful loss, but roughly half of Zepbound. The two drugs have not been compared head-to-head; cross-trial differences in population, baseline BMI, and lifestyle co-intervention apply, but the magnitude gap is large enough that the ranking is robust.

Why would I pick the oral pill instead of the injection?

Three patient profiles: needle-averse patients (a significant minority refuse injections even after counseling), frequent travelers (an oral pill avoids cold-chain storage and TSA screening of injectable pens), and dose-flexibility seekers (a daily oral dose can be paused or resumed more responsively than a weekly subq with a 7+ day half-life). Foundayo also lacks the strict fasting protocol that Rybelsus (the older oral GLP-1) requires — take Foundayo with or without food, any time of day.

Does Foundayo really work without food restrictions?

Yes — and this is the defining advantage over Rybelsus. Foundayo is a non-peptide small-molecule designed for oral absorption from the outset, so it does not require an empty stomach or a 30-minute post-dose fast. Lilly's pharmacokinetic studies showed bioavailability and exposure were not meaningfully affected by food (Ma 2024 Diabetes Ther; orforglipron PK was the original molecule's signature feature). ATTAIN-1 and ACHIEVE-1 protocols allowed dosing with food, and efficacy still hit pivotal endpoints. This is a real selling point for daily-adherence patients.

Are both FDA-approved for weight loss?

Yes. Zepbound was FDA-approved for chronic weight management in adults with obesity (BMI ≥30) or overweight (BMI ≥27 with weight-related comorbidity) in November 2023, with an additional indication for moderate-to-severe obstructive sleep apnea added in December 2024 (Malhotra 2024 NEJM). Foundayo was FDA-approved in 2025 for both chronic weight management and type 2 diabetes based on the ATTAIN-1 and ACHIEVE-1 pivotal trials. Both are on-label for obesity prescribing — no off-label workaround needed for either.

Which is cheaper?

Zepbound currently has the more transparent cash-pay tier: $349/mo starter and $499/mo maintenance for single-dose vials via LillyDirect direct-pay. Foundayo launched in 2025 and Lilly's direct-pay tier is rolling out — current cash pricing varies by pharmacy and is not yet uniformly published at the LillyDirect Zepbound level. Both are branded specialty drugs without generics. Commercial insurance coverage is the larger variable: many plans now cover Zepbound for obesity, while Foundayo's formulary status is still being negotiated payer-by-payer through 2026.

Can I switch between Zepbound and Foundayo?

Clinically, yes — different molecular classes (tirzepatide is a peptide dual agonist; orforglipron is a non-peptide GLP-1-only small molecule), so there is no dose-equivalence table. Standard practice when switching either direction is to stop the prior drug and restart the new one at its lowest titration dose (Zepbound 2.5 mg weekly, or Foundayo 3 mg daily), then titrate up every 4 weeks per the FDA label. Expect a brief GI tolerability period during re-titration. Most patients who switch from Zepbound to Foundayo do so because they cannot tolerate or cannot access injections — not because efficacy is equivalent (it is not).
