No direct head-to-head RCT exists yet, so cross-trial comparison comes with caveats — but the magnitude favors Wegovy modestly. For most patients, the 3-point gap is smaller than the gap between either drug and lifestyle alone.
Should I pick the pill if I'm needle-averse?
Yes — Foundayo is a strong pick for injection-averse patients. Unlike Rybelsus (oral semaglutide), which only delivers about 3-5% weight loss because peptides poorly survive oral absorption, Foundayo is a non-peptide small molecule designed for oral bioavailability from the start. You give up roughly 3-4 percentage points of efficacy versus Wegovy but avoid weekly injections entirely. For patients who cite injection fear as a barrier to starting GLP-1 therapy, Foundayo is a meaningfully better oral option than Rybelsus.
Does Foundayo need fasting like Rybelsus does?
No. Foundayo's non-peptide pharmacology means food and water do not meaningfully affect absorption — you take it any time of day, with or without meals, per the FDA prescribing information (DailyMed SetID 8ac446c5-feba-474f-a103-23facb9b5c62). This is a major practical advantage over Rybelsus, which requires an empty stomach, no more than 4 oz of plain water, and a 30-minute fast afterward. Wegovy is a weekly injection with no food-timing requirement either. So on the food-restriction axis, Foundayo and Wegovy tie — both fit easily into normal eating patterns.
Are both covered by insurance?
Wegovy coverage is widespread where employer plans include an obesity benefit — most commercial plans cover it after BMI ≥30 (or ≥27 with comorbidity) plus prior auth. Medicare does not cover Wegovy for weight loss but does cover it for cardiovascular risk reduction in patients with obesity and established CVD (per the SELECT indication). The FDA added cardiovascular risk reduction to the Wegovy label in March 2024. Foundayo's dedicated cardiovascular outcomes trial is still enrolling and has not read out — until it does, the drug has no MACE-reduction evidence. If proven CV protection drives your choice, Wegovy wins.
How do the side effects compare?
Both are dominated by gastrointestinal effects — nausea, diarrhea, constipation, vomiting — typical of GLP-1 receptor activation regardless of route. In STEP-1, Wegovy 2.4 mg produced nausea in roughly 44% of patients; in ATTAIN-1, Foundayo 17.2 mg produced nausea in roughly 20-30% of patients (dose-dependent). Both carry the FDA boxed warning for thyroid C-cell tumors based on rodent data. Titration is gradual for both to minimize tolerability problems. Patients tend to find oral GLP-1 GI symptoms slightly easier to manage than injectable peaks, but the underlying receptor activation drives the same physiology.
