Is Saxenda or Rybelsus FDA-approved for weight loss?

Only Saxenda. Saxenda (liraglutide 3.0 mg) is FDA-approved for chronic weight management in adults (Dec 2014) and adolescents 12 and older (Dec 2020). Rybelsus is FDA-approved only for type 2 diabetes (Sept 2019); any weight loss with Rybelsus is off-label.

Which produces more weight loss, Saxenda or Rybelsus?

Saxenda produces more weight loss in head-to-head data. PIONEER 4 directly compared oral semaglutide 14 mg vs liraglutide 1.8 mg (the T2D dose, not the 3.0 mg weight-loss dose) and found similar modest weight changes in patients with diabetes. At the FDA-approved 3.0 mg weight-loss dose, Saxenda achieves -8.0% TBWL over 56 weeks (SCALE Obesity), while Rybelsus produces roughly -4% to -5% body weight at 14 mg.

Why does Rybelsus require fasting?

Rybelsus contains SNAC (sodium N-(8-[2-hydroxybenzoyl] amino) caprylate), an absorption enhancer that only works on an empty stomach. Patients must take Rybelsus with no more than 4 oz of plain water at least 30 minutes before the first food, drink, or other oral medication. Skipping the protocol reduces absorption dramatically.

Are both drugs daily?

Yes. Both Saxenda and Rybelsus are taken once daily. Saxenda is a subcutaneous injection (any time of day, with or without food). Rybelsus is an oral tablet that must be taken fasting in the morning.

Do both have the same safety warnings?

Yes. Both Saxenda and Rybelsus carry the same boxed warning for thyroid C-cell tumors (medullary thyroid carcinoma and MEN 2 syndrome) and are contraindicated in patients with a personal or family history of those conditions. The most common adverse events for both are gastrointestinal (nausea, vomiting, diarrhea, constipation).

Can a teenager take Saxenda or Rybelsus?

Saxenda is FDA-approved for adolescents ages 12 and older with obesity (since December 2020) and is the only daily Novo Nordisk weight-loss option with pediatric labeling. Rybelsus is approved for adults only.
